Guidelines to Late Withdraw
from a Prior Semester*

Part I
- Request permission to late
withdraw from specific courses (including the section
numbers) for a specific semester or summer term.
- Accompanying the properly
completed petition must be a drop slip for each class
and/or lab. Instructor's signature on the drop slip
is required and attached statement of support may improve
chances of approval.
Part II
- State the reason for the lateness
of your request.
- If your request is selective,
explain why you want to withdraw from only that/those
course(s) rather than a complete withdrawal from all
courses of that semester.
- Provide written
documentation (i.e., a letter from doctor or
employer, a medical statement, a newspaper clipping),
making sure pertinent dates are clearly indicated.
Written support from the instructor would be beneficial,
but may not be possible if instructor has left the
campus. In this case a departmental response is expected.
Without significant documentation positive response to
your request is unlikely.
- After you have met with an
advisor, discussed the above guidelines and completed the
petition, deliver the petition to an advisor in your
college/school to check for completeness.
- Follow-up is your
responsibility. Your advisor will inform you of the
method in which the decision will be communicated to you.
If it is unclear as to how or when you will be informed,
call your college/school office.
